What exactly is precision medicine? It is said that our country also has a tsinghua version of the definition, how are the two different?

  Professor luo guoan: precision medicine has three meanings. The first is the accurate diagnosis of diseases, the second is the precise application of drugs, and the third is the precise treatment scheme, including the evaluation of curative effect and prognosis. The original medicine is actually a translational medicine, which emphasizes the dual development of clinical research and basic research. The research results of basic medicine should be translated into clinical research. Precision medicine is a two-way conversion between basic research and clinical research, which aims to make medicine think more about personalized treatment. The precision medicine proposed by tsinghua university has been expanded in the American and European editions. There are two main points. First, precision medicine in the United States and Europe mainly proposes such serious diseases as cancer. In addition, the main technical means of precision medicine in the United States and Europe are genomics and proteomics. We have added traditional medicine, which is modern science and technology, to combine traditional medicine for precision medicine research.

  Professor xu hao (host) : could you give us a brief introduction on how to conduct research on traditional Chinese medicine under the concept of precision medicine?

Professor luo guoan: traditional Chinese medicine emphasizes holism. From the perspective of precision medicine, it combines people's diseases with those who are sick, that is, the combination of disease and syndrome. American and European precision medicine emphasizes human diseases. However, our traditional Chinese medicine emphasizes the use of evidence to represent the sick.   Professor xu hao (host) : as a new medical model, precision medicine is a more accurate positioning and sublimation of individualized medicine, which has something in common with TCM syndrome differentiation. TCM syndrome differentiation is the process of classifying patients into different syndromes by collecting all the data about diseases. It is another way of classifying patients in addition to the disease classification under the guidance of the overall concept, which is a beneficial supplement to precision medicine. How to combine the two in the future is worth further study. In addition, the study of polymorphism of genes related to cardiovascular diseases has been a hot topic in the field of "precision medicine" of traditional Chinese medicine in recent years.

  Prof. Xue mei: gene polymorphism has been a hot topic in recent years, especially in the cardiovascular field. Although the diagnosis and treatment of cardiovascular diseases have made great progress in precision medicine, it is still difficult to find the most specific and individualized diagnosis and treatment targets for cardiovascular diseases such as coronary heart disease, hypertension and arrhythmia, which are generally recognized as multi-gene diseases. The results of different patient groups and genes are not consistent, and the mutation rate of gene polymorphism varies greatly among different ethnic groups. Since 2003, the research group headed by academician Chen keji has studied the correlation between gene polymorphism and coronary heart disease and blood stasis syndrome from the perspective of platelet membrane glycoprotein, which plays an important role in the process of platelet activation and thrombosis. The results showed that the risk factors of chronic stable angina were independent, especially those with blood stasis syndrome. However, our classic blood-activating and blood stasis drugs, xuefu zhuyu oral solution, have a good effect on the improvement of aspirin resistance. As for the precision medicine of coronary heart disease and the research on the combination of disease syndromes of coronary heart disease, I think it should be an organic combination. This requires each of our researchers, in the research process, to carry out a real in-depth thinking and combination based on the basic theory of traditional Chinese medicine and western medicine.

  Academician Chen keji: I think precision medicine is guided by curative effect. In the aspect of traditional Chinese medicine, precision medicine is combined with disease evidence. Traditional Chinese medicine treats syndrome, which is different from western medicine. Western medicine emphasizes the micro, while Chinese medicine emphasizes the macro. We learn from precision medicine, but we also need to understand and grasp the whole, and combine the two. For example, hypertension, western medicine has molecular analysis, genetic analysis, some genetic, some non-genetic; The precision medicine we study in Chinese medicine can be reflected in the types of different syndromes, such as hyperactivity of liver Yang and Yin deficiency of liver and kidney. We should combine macroscopical and microcosmic factors with disease syndromes.

  Professor pu jielin: precision medicine was proposed in Europe and America, mainly for single genes. Most diseases, however, are complex, with multiple factors contributing to the propensity to develop a disease. The earliest development of traditional Chinese medicine was individualization. Although there was a major prescription for the same disease syndrome, different patients had to add or subtract drugs on the basis of the original prescription.

Professor luo guoan: there are two defects in western precision medicine. The first is fragmentation. Secondly, we lack the guidance of the overall concept and theory. We suggest that the precision medicine in our country, combined with the characteristics of traditional Chinese medicine, should be led by the combination of Chinese and western medicine to carry out more project plans.

  Professor xu hao (host) : with the development of precision medicine in western medicine, it has been extended to various disciplines, bringing great challenges to TCM, but also an opportunity. TCM syndrome differentiation is actually a good complement to the defects of single gene research in precision medicine in western medicine. Guided by the concept of precision medicine and combined with the study of disease syndromes, it is expected to make more valuable academic achievements by grasping the advantages of the overall macro and dynamic system of TCM.
